What BMI is and what it isn't

BMI is body mass divided by height squared. It is a population-level screen designed for sedentary adults of average build. It does not distinguish fat from muscle.

An active man with significant muscle mass will routinely register 'overweight' or even 'obese' by BMI while carrying a healthy body-fat percentage. The reverse is also true: a sedentary man within a 'normal' BMI can carry excess visceral fat (the 'TOFI' pattern — thin outside, fat inside).

Better metrics for active men

Waist circumference is the cheapest, most predictive single metric. Under 37 inches is excellent for most men; 37–40 is average; above 40 correlates with cardiometabolic risk regardless of BMI.

Body-fat percentage by DEXA or 7-site calipers is the gold standard. 12–18% is athletic; 18–22% is healthy; above 25% warrants attention.

Why BMI still appears on labs

It is simple, free, and tracks population trends. Use it as one data point, not as your primary metric.

Is BMI accurate for muscular men?
No. BMI cannot distinguish lean mass from fat. Use waist circumference and body-fat percentage instead.
What is a healthy BMI for men?
18.5–24.9 is the WHO 'normal' band, but a fit man at 26 with low body fat is healthier than a sedentary man at 23 with high body fat.
Does BMI correlate with testosterone?
Loosely, via body fat. Visceral fat — not BMI itself — is the relevant driver of aromatase and lower free testosterone.
